Le dispositif ne remplace pas anatomiquement la valve native, mais d\u00e9montre qu\u2019une proth\u00e8se m\u00e9canique peut assurer une fonction valvulaire chez l\u2019humain. Cette \u00e9tape figure dans l\u2019<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7525892\/\" target=\"_blank\" rel=\"noopener\"><strong>histoire des premi\u00e8res proth\u00e8ses valvulaires<\/strong><\/a>.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\"><em>Valve de Hufnagel (1952), l\u2019un des dispositifs fondateurs de l\u2019ing\u00e9nierie valvulaire moderne. \u00a9<\/em> <a href=\"https:\/\/academic.oup.com\/pmj\" target=\"_blank\" rel=\"noopener\"><strong><em>Postgraduate Medical Journal<\/em><\/strong><\/a> <em>\/ BMJ \u2014 1956<\/em><\/p>\n<\/div><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">La circulation extracorporelle permet ensuite le remplacement direct de la valve. En 1960, <strong>Dwight Harken<\/strong> r\u00e9alise un remplacement aortique orthotopique avec une proth\u00e8se m\u00e9canique \u00e0 bille et cage. La m\u00eame ann\u00e9e, <strong>Albert Starr<\/strong> et l\u2019ing\u00e9nieur <strong>M. Les architectures \u00e9voluent avec les mat\u00e9riaux : alliages m\u00e9talliques, pyrocarbone et rev\u00eatements sont s\u00e9lectionn\u00e9s pour leurs propri\u00e9t\u00e9s m\u00e9caniques, leur r\u00e9sistance \u00e0 la fatigue et leur comportement vis-\u00e0-vis du sang.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La valve m\u00e9canique reste associ\u00e9e \u00e0 une forte durabilit\u00e9, avec une anticoagulation au long cours n\u00e9cessaire chez la plupart des patients pour r\u00e9duire le risque thromboembolique.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Le tissu entre dans l\u2019ing\u00e9nierie valvulaire<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Les bioproth\u00e8ses utilisent des valves porcines ou du p\u00e9ricarde bovin trait\u00e9s et stabilis\u00e9s pour constituer les feuillets. Chez <a href=\"https:\/\/www.cardiovascular.abbott\/fr\/fr\/home.html\" target=\"_blank\" rel=\"noopener\"><strong>Abbott<\/strong><\/a>, la gamme de valves biologiques chirurgicales<strong> <\/strong><a href=\"https:\/\/www.cardiovascular.abbott\/us\/en\/hcp\/products\/structural-heart\/surgical-valve-solutions\/epic-tissue-valve.html\" target=\"_blank\" rel=\"noopener\"><strong>Epic\u2122<\/strong><\/a> illustre cette approche avec une bioproth\u00e8se \u00e0 tissu porcin con\u00e7ue autour des probl\u00e9matiques de calcification et de durabilit\u00e9.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le d\u00e9veloppement des bioproth\u00e8ses introduit une autre variable : le vieillissement biologique. Calcification, fatigue m\u00e9canique, d\u00e9chirures et d\u00e9g\u00e9n\u00e9rescence structurale peuvent progressivement modifier les propri\u00e9t\u00e9s du tissu implant\u00e9. Les m\u00e9canismes de d\u00e9gradation des <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/8186572\/\" target=\"_blank\" rel=\"noopener\"><strong>valves biologiques de substitution<\/strong><\/a> sont \u00e9tudi\u00e9s depuis plusieurs d\u00e9cennies.<\/p>\n\n\n\n<div class=\"wp-block-media-text is-stacked-on-mobile\" style=\"grid-template-columns:22% auto\"><figure class=\"wp-block-media-text__media\"><img loading=\"lazy\" decoding=\"async\" width=\"205\" height=\"136\" src=\"https:\/\/think.green\/wp-content\/uploads\/2026\/08\/image-19.png\" alt=\"\" class=\"wp-image-16260 size-full\"\/><\/figure><div class=\"wp-block-media-text__content\">\n<p class=\"has-small-font-size wp-block-paragraph\"><em>Valve Epic\u2122 \u2014 bioproth\u00e8se chirurgicale \u00e0 feuillets porcins d\u00e9velopp\u00e9e par Abbott, con\u00e7ue pour int\u00e9grer les possibilit\u00e9s de prise en charge ult\u00e9rieure dans la conception du dispositif. \u00a9 Abbott<\/em><\/p>\n<\/div><\/div>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Du c\u0153ur ouvert au cath\u00e9ter<\/strong><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>La proth\u00e8se emprunte la voie art\u00e9rielle<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Au d\u00e9but des ann\u00e9es 1990, les recherches sur l\u2019implantation transcath\u00e9ter font \u00e9merger une nouvelle configuration : une bioproth\u00e8se mont\u00e9e sur une armature m\u00e9tallique, comprim\u00e9e sur un syst\u00e8me de d\u00e9livrance puis achemin\u00e9e jusqu\u2019\u00e0 la valve native par voie vasculaire. La voie transf\u00e9morale deviendra ensuite la principale voie d\u2019acc\u00e8s au TAVI.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le 16 avril 2002, \u00e0 Rouen, le <a href=\"https:\/\/institut-alaincribier.fr\/alain-cribier-pionnier-mondial-du-traitement-percutane-des-maladies-valvulaires\/\" target=\"_blank\" rel=\"noopener\"><strong>Pr Alain Cribier<\/strong><\/a> r\u00e9alise la premi\u00e8re implantation humaine d\u2019une <strong>valve aortique transcath\u00e9ter<\/strong> chez un patient atteint d\u2019une st\u00e9nose aortique s\u00e9v\u00e8re consid\u00e9r\u00e9 comme inop\u00e9rable. L\u2019intervention est aujourd\u2019hui consid\u00e9r\u00e9e comme le point de d\u00e9part clinique du <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31383557\/\" target=\"_blank\" rel=\"noopener\"><strong>TAVI moderne<\/strong><\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La proth\u00e8se doit alors r\u00e9pondre simultan\u00e9ment \u00e0 plusieurs contraintes : faible profil d\u2019insertion, r\u00e9sistance \u00e0 la compression, d\u00e9ploiement contr\u00f4l\u00e9, ancrage dans une valve souvent calcifi\u00e9e, stabilit\u00e9 et performance h\u00e9modynamique.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"900\" height=\"506\" src=\"https:\/\/think.green\/wp-content\/uploads\/2026\/08\/image-21.png\" alt=\"\" class=\"wp-image-16264\" style=\"aspect-ratio:1.775811209439528\" srcset=\"https:\/\/think.green\/wp-content\/uploads\/2026\/08\/image-21.png 900w, https:\/\/think.green\/wp-content\/uploads\/2026\/08\/image-21-300x169.png 300w, https:\/\/think.green\/wp-content\/uploads\/2026\/08\/image-21-767x431.png 767w\" sizes=\"(max-width: 900px) 100vw, 900px\" \/><\/figure>\n\n\n\n<p class=\"has-text-align-center has-small-font-size wp-block-paragraph\"><em>Pr Alain Cribier (1945\u20132024), pionnier du traitement percutan\u00e9 des valvulopathies et figure fondatrice du TAVI. Depuis, plus de 4 millions de patients ont \u00e9t\u00e9 trait\u00e9s par TAVI dans le monde. \u00a9<\/em> <a href=\"https:\/\/institut-alaincribier.fr\/\" target=\"_blank\" rel=\"noopener\"><strong><em>Institut Alain Cribier<\/em><\/strong><\/a><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>L\u2019anatomie devient une donn\u00e9e de conception<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Le scanner cardiaque caract\u00e9rise l\u2019anneau aortique, la racine aortique, les calcifications, la hauteur des ostia coronaires et les voies d\u2019acc\u00e8s. Ces donn\u00e9es d\u00e9terminent la taille de la proth\u00e8se, sa position et la faisabilit\u00e9 de l\u2019implantation.
